When Extraction Is Your Best Option

Orthodontists and dentists use advanced procedures to treat and save teeth. But sometimes, a tooth just can’t be saved and needs to be extracted. Sometimes it’s due to extensive decay, or infection, or serious injury. Or sometimes, to create a healthy bite, an extraction is necessary during orthodontic treatment.

When does an orthodontist recommend an extraction? Conditions which call for extraction include:

  • Incoming Wisdom Teeth

Not many people have enough space in their mouths for four wisdom teeth! And when there’s no room to erupt properly, wisdom teeth can become decayed or infected, push your other teeth out of place, and damage the roots of nearby molars. Extracting wisdom teeth protects your teeth and your bite alignment.

Usually, orthodontic treatment takes place before the wisdom teeth begin to erupt. But if yours start to make an appearance during treatment, your orthodontist and dentist will work together to schedule extraction without interfering too much with your orthodontic treatment plan.

  • Tooth and Jaw Structure

Dr. Perry and the team at Robert H. Perry Specialist in Orthodontics carefully plan your treatment based on examinations, X-rays, and/or 3D imaging. The goal is to align your teeth and jaws for an attractive smile and a healthy, functional bite. When overcrowding is severe, or if there is a serious malocclusion (bite problem), or if a particular tooth is unusually large or misshapen, your orthodontist might recommend an extraction.

Sometimes more room can be made for teeth with appliances such as palatal expanders or, in other cases, with the help of oral surgery. Extractions are only recommended when they are absolutely necessary.

  • Baby Teeth Which Aren’t Coming Out 

Baby teeth normally fall out in a predictable pattern. Front teeth first, canines and molars last. Baby teeth have small roots, and these roots begin to break down when the adult teeth underneath put pressure on them as they start to erupt. As the root gets smaller and smaller, the baby tooth gets looser and looser—until it falls out. 

Sometimes, though, the roots of primary teeth don’t break down, and the baby teeth stay right where they are. This means that the permanent teeth have to erupt someplace else—usually behind those baby teeth. This creates a double row of teeth, and your permanent teeth can become crooked or overlap as they try to fit in. Extracting stubborn baby teeth allows your adult teeth to come in just where they’re supposed to. Because of their small roots, extracting primary teeth is usually simple and straightforward, and can be done in your dentist’s office. 

  • Too Many Teeth

Most of us have 32 adult teeth, including wisdom teeth. It’s rare, but sometimes an extra, or supernumerary, tooth develops. Your jaw is most probably not equipped to accommodate any extras, so, in this case, the extra tooth or teeth are removed to give the rest of the teeth the space they need to avoid overlapping or crowding.

When your orthodontist recommends an extraction, it’s because it’s important for your dental health. Dr. Perry will refer you to a dentist or a specialist like a periodontist or oral surgeon, who will: 

  • discuss the extraction procedure with you, including sedation options
  • numb the area 
  • gently loosen and then extract the tooth
  • provide options for pain management after the procedure
  • give you detailed instructions for aftercare
  • work with your orthodontist to keep your treatment on schedule

What causes crooked teeth?

Crooked teeth, more correctly called malocclusions, have reached epidemic proportions in the late 20th and early 21st centuries. According to the American Association of Orthodontists, approximately 80 percent of American teenagers are currently undergoing orthodontic treatment. Although advances in orthodontic devices and increased availability of such devices explain part of this increase, it still means there are a lot of crooked teeth in the world.

The theories about what is causing so many crooked teeth range from the ridiculous to the scientific. For years, oral health professionals believed that crooked teeth were an evolutionary result of the change in Western diets from raw, wild foods to soft, processed foods. That theory has since been debunked.

The truth is that crooked teeth can be caused by a number of things. Crooked teeth can be an inherited trait. Parents with crooked teeth and malformed jaws are more likely to have children with malocclusions. Ill-fitting or poorly-executed dental restorations, such as fillings and crowns, can also cause teeth to become crooked. Baby teeth that fall out early, gingivitis, and even a jaw that is too small to accommodate all of a person’s adult teeth are additional causes of crooked teeth. In addition, thumb sucking and the use of a pacifier for too long can contribute to crooked teeth.

Orthodontic Treatments

It’s not unusual for a patient to be unaware of the range of services that orthodontists can provide for their patients. Knowing which services Dr. Perry and our team perform can help you better understand your options and why we might select a particular method of treatment.

Both orthodontists and dentists care about good oral health, but they approach treatment in differing ways. You know that dentists clean teeth and treat gum disease, tooth decay, toothaches, and other oral health problems.

But what does your orthodontist do besides help straighten teeth with the help of braces? Orthodontists are commonly known to help fix or realign crooked teeth. Many of the patients at Robert H. Perry Specialist in Orthodontics come in for appointments that relate to their braces.

Dr. Perry and our team recommend that children be seen around the age of seven to evaluate their potential orthodontic needs. Common problems may include overcrowding of teeth, large gaps or spaces, and overbites or underbites, among other things.

In order to address these common problems, we offer several methods of treatment besides standard braces and retainers:

  • Space maintainers can be used to fill the spaces left by missing baby teeth so other teeth don’t shift and occupy the adult tooth’s location.
  • Jaw repositioning appliances, sometimes known as splints, are used to reposition the upper and lower jaw bones correctly.
  • Lip and cheek bumpers can also be used to avoid having to pull teeth. These bumpers are placed in the mouth so the patient’s lips or cheeks don’t put pressure on specific teeth.
  • A more common appliance that orthodontists use is expanders. If your mouth is crowded, expanders will be placed on the curve of the upper and/or lower jaw(s) in order to make room for teeth to be properly aligned.
  • As a last resort, an orthodontist may turn to headgear. This is normally provided to slow down the growth of the jaw. It must be worn a number of hours each day.

The Best Brush of the Day

Imagine that you’re only going to brush your teeth once tomorrow. Don’t worry, we know you would never skimp on your dental hygiene like that, but let’s just pretend for a moment. When would be the best time to brush? When you wake up? During the day? Or perhaps before you go to bed?

Actually, whenever you choose to brush, you’ll receive important overall dental benefits as well as specific benefits tied to the time of day. Let’s explore your daily schedule to see why.

Brushing in the Morning

Brushing when you first jump out of bed produces several positive results.

  • Cleaning plaque from your teeth

Plaque is a sticky film made up of oral bacteria, food particles, and saliva. As you sleep, these oral bacteria multiply and produce acids which attack the minerals in your enamel, leaving weak spots which, over time, can become cavities. Brushing removes these bacteria and acids from your enamel before they cause serious harm.

Moreover, plaque hardens if it’s left undisturbed, turning into tartar in a relatively short time. And once plaque becomes tartar, it must be removed by a dental professional. Brushing first thing in the morning removes this plaque buildup and helps prevent tartar from forming.

  • Fresh breath

That bacterial growth we mentioned? It’s also responsible for morning breath. If nothing else, brushing when you wake up means greeting a fresh day with fresh breath, and that’s reason enough to pick up your brush in the morning.

Brushing During the Day

Brushing after meals and snacks also has a lot to recommend it.

  • “Leftovers” lead to cavities

Foods, especially foods rich in sugar and carbohydrates, are converted by oral bacteria into acids which weaken enamel and lead to cavities. When food particles remain in the mouth after a meal, bacteria have more time and more fuel to manufacture these acids.

  • Acidic foods also affect your teeth

If you have eaten something acidic, such as citrus fruits, sodas, or pickled anything, the acids from these foods can temporarily weaken the mineral strength of your enamel. But brushing immediately after eating or drinking acidic foods can damage weakened enamel. Better to rinse well with water and brush after half an hour or so.

  • When you wear braces

One of the first things you discover when you get your braces is that you might need to brush more often. In fact, it’s best to brush after every meal and even every snack while you’re in braces.

Why? First, because no one wants to smile with food particles sticking to brackets and wires. Even more important, though, the filmy plaque which sticks to your enamel can be harder to remove with those brackets and wires in the way. Since plaque causes weakened enamel and cavities, brushing thoroughly is more important than ever when you wear braces.

  • When you wear aligners

Wearing clear aligners means you don’t need to worry about food trapped in brackets or cleaning around wires. After all, you take them out when you eat. But this doesn’t mean you are home free. Brushing after every meal is also a good idea when you wear aligners.

Our teeth have an organic way to help wash away food particles, acids, and bacteria between brushings—saliva! Your aligners, while covering your teeth, decrease their exposure to saliva. It’s really important, then, to make sure you brush after eating. Otherwise, food particles and acids which remain on your teeth after eating are trapped in your aligners, increasing the risk of enamel erosion and decay.

Brushing at Night

Growing up, you probably received regular reminders to brush before bedtime—for several really good reasons:

  • Saliva production slows while you sleep

During the day, saliva helps to wash away food particles and neutralize acidity in our mouths. It also contains proteins and minerals which help keep tooth enamel strong. But as we sleep, saliva production slows dramatically, and our bodies can’t remove bacteria and acids as effectively.

  • Food particles fuel bacterial growth

If you haven’t brushed since morning, you’ve accumulated a whole day’s worth of food particles from meals and snacks. Remember, oral bacteria use the sugars and carbs we eat as fuel to produce the acids which attack our tooth enamel throughout the night.

  • Brushing helps prevent both of these problems

Brushing your teeth before bed not only cleans away the accumulated food particles of the day, but also eliminates the plaque and bacteria which would have a much easier time sticking to your teeth without that daytime saliva flow to wash them away.

So, When’s the Best Time to Brush?

In the morning, during the day, at night—there are solid advantages to brushing any time of day. The question isn’t so much when to brush as how often you should brush.

While many dental professionals consider brushing before bedtime as the most important brush of the day, brushing at least two full minutes, at least twice during a 24 hour period, is a necessity for basic dental hygiene, along with flossing at least once a day.

When you’ve been eating sugary snacks, when you’re showing signs of gingivitis or getting more than your share of cavities, when you want to reduce the chance of plaque and tartar buildup, or when you simply want to make sure you’re doing everything you can to maintain your overall dental health, brushing after meals is also highly recommended.

And when you wear braces or aligners, frequent brushing (and flossing) is the very best way to make sure your teeth stay clean and cavity-free.
