Just What Is Plaque?

From the time you were small, you’ve been warned about the dangers of plaque. Why? Because:

  • It’s an unpleasant film that sticks to your teeth
  • It causes cavities
  • It causes gum disease
  • It can cause extra problems when you wear braces

And really, do we need to know much more than this to motivate us to brush? But if you’re in a curious mood, you might be wondering just how this soft, fuzzy film accomplishes all that damage. Let’s take a closer look at the sticky problem of plaque.

How does plaque form?

We live with hundreds of species of oral bacteria, most of which are harmless, and some of which are actually beneficial. But when our oral ecosystem gets out of balance, problems can occur. For example, without regular and thorough brushing and flossing, we start to build up plaque.

Plaque starts forming within hours of your last brushing. And even though plaque fits the very definition of “seems to appear overnight,” this biofilm is actually a complex microbial community with several different stages of development.

  • It starts with saliva.

Saliva is vital to our oral health, because it keeps us hydrated, washes away food particles, neutralizes acids in the mouth, and provides minerals which keep our enamel strong. Saliva also contains proteins, which help form a healthy, protective film on the tooth surface. This film is called a pellicle.

  • Bacteria attach to the pellicle.

There are species of oral bacteria that are able to attach themselves to the pellicle film within hours of its formation. As they become more firmly attached, they begin to grow and divide to form colonies, and are known as the early colonizers of the plaque biofilm.

  • A complex biofilm forms.

If you’ve skipped brushing for a few days (please don’t!), you’ll notice a fuzzy, sometimes discolored film on your enamel—that’s a thriving plaque community, and it only takes a matter of days to go from invisible to unpleasant.

If you’re not removing plaque regularly, it can harden further and become tartar. And once you have tartar buildup, you’ll need the care of a dental professional to remove it.

  • What happens if we ignore plaque and tartar?

We get cavities and gum disease.

How does plaque cause cavities?

  • The bacteria in plaque, like all organisms, need nutrients.

Our normal oral environment and the food in our everyday diets provide the nutrients plaque needs. And, as we mentioned above, certain types of oral bacteria convert these nutrients into acids. Foods such as carbohydrates, starches, and sugars are most easily converted into acids, which is why we recommend that you enjoy them in moderation.

  • The biofilm promotes acid production.

Within the plaque film, anaerobic bacteria (bacteria which don’t use oxygen) convert sugars and starches into acids. As the plaque film becomes denser, it blocks acid-neutralizing saliva and oxygen from reaching these bacteria close to the tooth’s surface, creating an ideal environment for the bacteria to produce their acid waste products.

  • Acids attack enamel.

The sticky nature of plaque keeps these acids in contact with tooth enamel, where, over time, acids dissolve minerals in enamel, weakening the mineral structure of the tooth.

How does plaque cause gum disease?

  • Bacteria cause inflammation and gingivitis.

The bacteria in plaque irritate the delicate tissue of the gums, which causes an inflammation response which can leave your gums swollen, red, bleeding, or tender. This early form of gum disease is gingivitis. Fortunately, good dental care and careful brushing and flossing can usually prevent and even eliminate gingivitis.

  • Plaque and tartar can lead to periodontitis.

When plaque and tartar build up around and below the gumline, the gums pull away from the teeth, leaving pockets where bacteria collect, leading to infection as well as inflammation. Infections and constant inflammation not only harm gum tissue, they can destroy the bone supporting the teeth. This serious gum condition is periodontitis, and should be treated immediately to avoid further infection and even tooth loss.

How does plaque affect orthodontic patients?

  • Plaque collects around your braces.

Braces provide plenty of spots for plaque to hide from your brush. If you aren’t extremely diligent with your brushing and flossing, plaque collects near brackets, wires, and bands—all those spots that a brush and floss find difficult to reach.

  • Plaque promotes demineralization

The demineralization process we mentioned above can cause white spots on teeth (decalcification), where minerals have dissolved. Sometimes these spots can be treated, and sometimes they are permanent. They can become quite sensitive, and may lead to cavities.

Careful brushing and flossing around your braces will help eliminate the plaque that can cause demineralization near brackets. Ask Dr. Perry about the tools and the brushing and flossing techniques which will give you the best results.

How do we fight plaque?

From the time you were small, you’ve learned how to fight plaque:

  • Brush at least twice a day for two minutes, and be sure to brush all of your tooth surfaces and around the gumline.
  • Floss to remove plaque from between the teeth and near the gumline.
  • See your dentist as recommended for a thorough professional cleaning.

The Evolution of Braces

Did you know that even in ancient times, people wanted to improve the look and function of their smiles? Robert H. Perry Specialist in Orthodontics thinks of modern orthodontic appliances as sleek, efficient technology, but this was not always so! Take a look at the highlights in the evolution of braces.

Ancient Times: From Greece to Rome

  • According to The Angle Orthodontist, Aristotle and Hippocrates first thought about methods for straightening teeth between 400 and 300 BC.
  • The Etruscans, in what we now know as Italy, buried their dead with appliances that maintained spaces and prevented collapse of their teeth and jaws during life. Archaeologists have discovered mummified remains in various locations that have metal bands wrapped around the teeth.
  • A Roman tomb has also been discovered in which the teeth were bound with gold wire, including documentation on the wire’s use as a dental device.

18th Century: A French Development

  • The French dentist Pierre Fauchard is acknowledged as the father of modern dentistry. In 1728 he published a book that described various methods for straightening teeth. Fauchard also used a device known as a “blandeau” to widen the upper palate.
  • Louis Bourdet was another French dentist who published a book in 1754 that discussed tooth alignment. Bourdet further refined the blandeau and was the first dentist to extract bicuspids, or the premolar teeth between canines and molars, for the purpose of reducing tooth crowding.

19th Century: Orthodontics Defined

  • Orthodontics started to become a separate dental specialty during the early 19th century. The first wire crib was used in 1819, marking the beginning of modern orthodontics.
  • During this period, gold, platinum, silver, steel, gum rubber, vulcanite, and occasionally wood, ivory, zinc, and copper were used — as was brass in the form of loops, hooks, spurs, and ligatures.
  • Edward Maynard first used gum elastics in 1843 and E. J. Tucker began making rubber bands for braces in 1850.
  • Norman W. Kingsley published the first paper on modern orthodontics in 1858 and J. N. Farrar was the first dentist to recommend the use of force over timed intervals to straighten teeth.

20th Century: New Materials Abound

  • Edward Angle developed the first classification systems for malocclusions (misaligned teeth) during the early 20th century in the United States, and it is still in use today. Angle founded the American Society of Orthodontia in 1901, which was renamed the American Association of Orthodontists in the 1930s.
  • By the 1960s, gold was universally abandoned in favor of stainless steel.
  • Lingual braces were the “invisible” braces of choice until the early 1980s, when tooth-colored aesthetic brackets made from single-crystal sapphire and ceramics became popular

Today

As we arrive in the present, you need only look at your own braces to see how far we’ve come. Your treatment plan was probably created with a 3D digital model, and we’ve likely used a computerized process to customize your archwires. Perhaps you have clear aligners, self-ligating brackets, or highly resilient ceramic brackets with heat-activated wires.

Dr. Perry and our team will want you to make sure your teeth and gums are healthy before you begin. A complete dental exam will let you know if you have any dental conditions that need to be addressed before you get braces or aligners. Why be proactive? Because if you need dental work during your orthodontic treatment, it can cause delays in your treatment plan.

  • Cavities

Cavities grow over time and can eventually reach the inside of the tooth, causing infection and damaging the pulp. For this reason alone, it’s best to find and treat cavities before you get braces or aligners.

If you’re wearing braces when a cavity’s discovered, the situation gets a bit more complicated. Your dentist might be able to work around your braces if the cavity is easy to reach. But if a cavity’s near your brackets, Dr. Perry might have to remove a wire or bracket before treatment can start. This means scheduling different appointments to a) remove a section of your braces, b) repair your cavity, and c) replace your bracket and/or wire. See what we mean? Definitely more complicated.

  • Gum Disease

While serious gum disease is most common in adults, children and teens can suffer from early-stage gum disease. Your gum health affects your orthodontic treatment—and can be affected by it, too—so your gums need to be healthy before you begin wearing braces or aligners.

Early gum disease can frequently be reversed with careful brushing and flossing habits. Because advanced gum disease weakens the bone under the teeth, patients with this kind of gum disease will need dental or periodontal treatment before any orthodontic work.

  • A Clean Start

Once you’ve made sure your teeth and gums are healthy, there’s one more proactive step you can take for a better orthodontic experience. Whenever you’re at the dentist for an exam, it’s always a good idea to get a professional cleaning. It’s especially important when you’re getting—or wearing—braces.

Even with the most dedicated everyday brushing and flossing, plaque and tartar can build up, especially between and behind the teeth. A professional cleaning by an expert is just what you need to remove any plaque or tartar that you’ve missed before you start wearing braces or aligners.

Your beaming smile makes a sunny day even brighter—and that sunny day can return the favor! How does the sun affect your dental health? Let’s shine some light on the subject.

You may have heard how important calcium is to your body. Over 99% of the calcium in our bodies is used to form teeth and bones, providing strength and structure to the hardest parts of the body. But that’s not the only task on the calcium to-do list. Calcium is also necessary for:

  • muscles to work
  • nerves to carry messages
  • blood to deliver nutrients and oxygen
  • the release of hormones

When we don’t get enough calcium in our diets, our bodies start to draw the calcium they need for these critical functions from our teeth and bones. Over time, a severe calcium deficiency can lead to teeth that are brittle, or more vulnerable to decay, or loose due to weakened bone density in the jaw.

Keeping up with your calcium is especially important when you’re wearing braces or aligners. Calcium helps protect you from tooth decay and gum disease, which could delay your treatment. And you want your jaw bones strong and healthy as the bone tissue reforms and then stabilizes around your teeth as they move to their new, ideal positions.

We can get the calcium we need from a balanced diet. Dairy products are our most common source of calcium, and several servings of milk, cheese, or yogurt each day supply most of our needs. If dairy’s not on your menu, though, you can get calcium from other foods like broccoli, kale, collard greens, canned salmon, or sardines. Because calcium is so essential to our health, many foods are fortified with it, including fortified juices, bread, cereals—even tofu!

But there’s a snag here, and it’s a big one. We can’t really benefit from our careful consumption of calcium without the help of vitamin D. If calcium is the doorway to strong teeth and bones, vitamin D is the key which unlocks all of calcium’s benefits.

Without enough vitamin D, our bodies can’t absorb and make use of calcium. However, getting enough of this essential vitamin can be a problem. While we can get most of the vitamins we need from a balanced diet, there aren’t a lot of foods which are naturally rich in vitamin D.

So, how do we get this vital vitamin? The original option comes from nature.

  • Sunlight

Vitamin D is called “the sunshine vitamin” because of the unique chemical reaction which creates it. When the ultraviolet B rays of the sun reach special cells in our skin, they convert cholesterol in these cells to a basic form of vitamin D. These vitamin molecules travel through the body, picking up hydrogen and oxygen molecules in the liver and kidneys to become active vitamin D.

Since sunlight exposure is easy, available, and free, it seems like a simple solution to getting the vitamin D we need.

But we’ve understandably moved away from the days of basking in the sun for hours at a time. Over-exposure to UV rays damages the skin, causing premature aging and, worse, a much greater risk of skin cancer. If you enjoy time in the sun, talk to your doctor about safe sun exposure, which can depend on the part of the country you live in, the time of year, and your individual skin type.

And, if you need to avoid sun exposure, there are other opportunities for getting the vitamin D you need.

  • Diet

While there aren’t a lot of foods which are naturally rich in vitamin D, there are still a few options on the menu. Oily fish like salmon, tuna, and herring—and old-fashioned cod liver oil—are good sources of the vitamin. But most of us don’t live on a strict fish diet!

Instead, we can get the vitamin D we need from fortified foods. Vitamin D is commonly added to cow’s milk, soy milk, orange juice, and cereals to make sure the calcium in our diet is working effectively to keep our teeth and bones strong. Check out nutritional labels to discover whether your favorite grocery products contain vitamin D. And don’t forget to choose foods that are braces-friendly, as well!

  • Supplements

For hundreds of years, people who lived in climates and cities without a lot of sunshine suffered from the effects of vitamin D deficiency. Diseases like rickets, where the teeth and bones don’t form properly, were common in children. Adults who couldn’t absorb enough calcium suffered from weak teeth and bones.

Today, depending on age, allergies to sun or food products, dietary choices, and health conditions, some people are still at risk for vitamin D deficiency. If you’ve been diagnosed with this deficiency, your doctor might recommend a vitamin D supplement. (It’s important to take any supplement as directed, because taking too much vitamin D can be harmful, too.)

So, which form of vitamin D is best? Fortified foods? Supplements? Or is it that radiant original source—the sun? No need to worry.
